Create a form

The two-step wizard: title and questions, then page template.

What it is for

Build a questionnaire to send to your clients, for which Formtool generates the fillable PDF.

Prerequisites and plan

Be signed in. Free plan: 1 generated form per month; Pro: unlimited. Maximum 500 forms per account and 40 questions per form.

Steps

  1. Click “Forms” in the menu.
  2. Click “+ Create a new form”.
  3. Step “1. Title and questions”: fill in “Form title” (120 characters) and, if you wish, “Introduction message (optional)” (400 characters).
  4. Fill in each row: “Question” (200 characters), “Answer type”, “Required” box. The ↑ and ↓ arrows change the order, ✕ deletes the row, “+ Add a question” adds a row. Three rows are offered at the start (“Full name”, “Email” and an empty row); a row whose question is empty is ignored.
  5. Click “Next: choose a template →”.
  6. Step “2. Choose a template”: click the thumbnail you want (see “Page templates”). “← Edit the questions” goes back.
  7. Click “Submit and create the form”.

Expected result

The message “Form created: (title)” is shown with the “Download the PDF” button. The form is in “My library” and counts as 1 toward your monthly quota.

Tips

  • There is no screen for editing a form after it is created: to adjust it, recreate it or use the API (article “API: forms and PDF”).
  • Choice-type questions require 2 to 12 options, one per line.
